What is a CN22 and when do I need one?

It is the customs declaration that travels on a small international packet, under roughly 300 SDR in value and 2 kg in weight. Above those limits, or when your carrier asks, the larger CN23 on a separate sheet replaces it. The postal operator uses it to decide duty and whether the item may enter the country at all.

Is this the official form?

It is a filled CN22 layout, not a postal franchise. Print it and attach it, or copy the values onto the official form your carrier supplies: several carriers now also want the same data submitted electronically when you buy the postage. What customs authorities care about is that the declared contents, weights and values are accurate.

What should the item description say?

What the item actually is, in plain words a customs officer in the destination country understands. "Clothing" gets questioned; "2 cotton T-shirts, black" clears. Vague descriptions are the single most common reason a packet is opened or held.

Which value do I declare?

The real transaction value of the contents: what the buyer paid for the goods, excluding postage. Under-declaring to dodge duty is customs fraud, and the declared value is also what the insurance and the claim conversation lean on.
