Addresses in Poland are written in the Latin alphabet: copy the street line exactly as the recipient gave it.
Local conventions worth getting right in Poland
Street, house number and flat number sit together on one line, with the flat after a slash: ul. Słoneczna 12/5. The postal code takes the form 00-001, hyphenated and with no spaces inside it, followed by the town on the same line. Capitals throughout reduce sorting errors.
If nobody is home when the parcel reaches Poland
Poczta Polska counts the awizo in calendar days, not working days, so weekends and public holidays are inside the fourteen the recipient gets. A second awizo follows after seven days, and the item goes back to you at the end of the fourteenth. Storage can be extended by a further fourteen days for a small daily fee, which is worth mentioning to a buyer who will be away rather than letting the parcel travel back across a border.
Worked example: a parcel arriving in Warszawa

The last line is the destination country in English capitals, so the exporting post office can sort the item without reading the local script.
Poland uses postal codes. A valid one looks like 00-001, the code for Warszawa.
The national postal operator is Poczta Polska.
No state or province is used in a Poland address. The city line and postal code are enough.

Customs status
Poland is in the EU customs union. A parcel from another member state is in free circulation: no CN22, no declaration, no duty. The generator prints the customs block only on other routes.
Norway, Switzerland, Iceland and the United Kingdom are not in the union, despite the geography.

Is this an official postal label for Poland?
No, and no website can produce one. A carrier label carries a tracking number only Poczta Polska or a courier can issue against your account. This is an address and handling label: print it, tape it to the box, and add the carrier’s tracking label beside it at drop-off.
Does a parcel from another EU country to Poland need a customs form?
Not from another EU member state: that parcel is in free circulation, with no declaration and no duty. From anywhere outside the customs union, a CN22 is required below roughly 300 SDR and a CN23 on a separate sheet above it.
